Eligibility: This prize draw is open to participants who complete one of the following surveys: private physiotherapists, elite sports professionals, or current users of the A-FLEX device. Participants must be aged 18 years or over. Employees of the promoter, their family members, or anyone else connected with the prize draw may not enter.

1. Prize: The prize consists of one £250 Amazon voucher or an alternative voucher of the winner's choice, subject to availability. The prize is non-transferable and there are no cash alternatives.

2. Entry: Entry into the draw requires the completion of a survey. Participants can gain one additional entry by completing the final section of the survey showcasing the A-FLEX PRO ankle mobilising device. Further, participants can gain one extra entry for every person they refer who completes at least the first part of the survey. Referrals must mention the participant's name as how they heard about the survey. The Survey can only be completed once per person.

3. Data: Entry into the prize draw is contingent upon consenting to provide name and contact details (email), for the sole purpose of notifying the winner. Personal data will be used in accordance with the Data Protection Act 2018 and the promoter's privacy policy.

4. Draw: The prize draw will be conducted once 100 entries are received for each survey category. If the target is not met by 31st July, the draw will proceed on this date with the entries received. The winner will be chosen at random from all entries received across all surveys.

5. Notification: The winner will be contacted by email within 14 days of the draw date. If the winner cannot be contacted or does not claim the prize within 14 days of notification, we reserve the right to withdraw the prize from the winner and pick a replacement winner.

6. Promoter: The promoter of this prize draw is Azzurro Training Products Ltd (Trading as A-FLEX), with a registered office at Creative Industries Centre Wolverhampton Science Park, Glaisher Drive, Wolverhampton, England, WV10 9TG. The promoter reserves the right to cancel or amend the prize draw and these terms and conditions without notice in the event of a catastrophe, war, civil or military disturbance, act of God or any actual or anticipated breach of any applicable law or regulation or any other event outside of the promoter's control.

7. Liability: The promoter is not responsible for inaccurate prize details supplied to any entrant by any third party connected with this competition. No liability is accepted for any damage, loss, liabilities, injury, or disappointment incurred or suffered as a result of entering the prize draw or accepting the prize.

8. Jurisdiction: The prize draw and these terms and conditions will be governed by English law, and any disputes will be subject to the exclusive jurisdiction of the courts of England.
